I've lived all over the country as well and yes, some places are worse than others (don't get me started on the speeding and lane changing of Atlanta drivers...). And you are right about the elevator behavior. Maybe someone on this forum with a psychology background can enlighten us.
In a queue, you subconsciously are “protecting your space”, and trying to make sure no one cuts in front of you. Therefore, you will close up the gap between others.
Whereas in an elevator, you are trapped in a confined space, and not worried about protecting your space (like in a queue.) Therefore, you spread out as soon as possible.
Also, the person standing next to you in elevator are garlic for lunch, or is a smoker.
