So multiple failures of the same variety over a period of decades. Would that indicate a QA/QC issue during routine maintenance? Seems like something you'd want to correct more "permanently." Or is it akin to a roof leak, in that you don't know you have one until the water is pouring down the wall?I’m not saying it’s acceptable. I’m saying it’s not appropriate to imply that it’s the result of lowered standards since this same thing has happened multiple times before. Including back when standards were higher. We just didn’t always have cell phones and social media.
