It was mostly bad luck, but just some rough coincidences. The two boats colliding during Fantasmic certainly does raise some questions. I had always thought they were on rails, but evidently not.

So is it safe to say that when the soundtrack stops after 10 or so repetitions, the lights come on, then go off and the soundtrack starts back up that it's a true mechanical issue vice some guest accommodation?

No.

Should something take longer than allowed at load or unload - even with creep mode - a ride stop will be initiated. Should it be a handpac that triggers this the ride could E stop. Depending on the ride in question it can lead to various things happening, including what you describe.
