Tony invented something to allow a successfully surgery I'm sure.
It was the extremist he cured pepper then used it on himself for healing powers.

The whole plot twist with the Mandarin absolutely killed the movie for me! Movies take creative license from the source materials all the time, some better than others, so I'm not fanboy griping that they ruined the character or anything like that. My problem with what happened there was that the exact same plot device was used in Batman Begins AND The Dark Knight Rises!! "The villain we've been building up this entire time of the movie is really the pawn of another less interesting character the whole time! Bet you didn't see that one coming!" That is what I found insulting.

I really didn't mind the Mandarin twist. I thought Ben Kingsley went a little too over the top sometimes when he was in "actor" mode, but I still really liked it. I knew it was coming, though, so maybe that helped me. Somewhere in the middle while listening to him doing the Mandarin schtick, I was actually really glad that there wasn't another movie with some terrorist mastermind spewing that pseudo-intellectual BS. I would like to see an actual take on the Mandarin sometime in the future, but I thought this version fit with the tone of the universe.
