FYI...the new medallions are out and have been accessible since Jan 2015. However, the Challenges that are attached to the WDW races have different courses than the usual "Free 5K". With the "Free 5K" the start is at the bike rental area; with the challenge the start is now on the airstrip and the course markings are much easier to find/see/noticable.

Does every cruise that makes a stop at Castaway Cay have a 5k?

If the cruise stops at Castaway, and weather permitting, then yes! You can sign up at the Port Adventures Desk or at the main desk in the Lobby. This is a free 5K and the medallions have been redesigned this year.
