Our first night in Disney we have character meal booked at The Garden Grill. If is also my son's birthday, how do I go about ordering a birthday cake?

My guess is you should call them directly ahead of time to see what can be done.

I have had cake severed for both my DS and DW birthdays always without a hitch. On my DS's birthday we ate at the California Grill, they severed the cake and sang happy birthday. Then a little girl from another table came over said "Happy Birthday" to my son and asked if she could have some cake. I think her parents wanted to murder here but my DW and I thought it was wonderful and she got her cake.
Does anyone know if you can order online?

No you can only do by phone or email just depends on the location but no online ordering.

As @sxeensweet noted, you can't order online for Epcot. MK is the only one with email cake ordering (and even some of that process is handled by phone).
