No, none have been lost or stolen. They have procedures to match the child to the parents. They are wonderful and I would not worry.
this may help. http://allearsnet.com/pl/childfaq.htm
I have also heard over the years that there are "undercover" cast members throughout the parks watching out for lost children. As an aside note : In the World of Disney there are no lost children, only misplaced parents !!

As I recall (it's been a looong time) both child and parent have armbands that they're given on check-in. Then, you had to show photo ID in order to sign out your child. Or perhaps I'm thinking of ski school. I suspect WDW follows a similar procedure.
As an aside, this was definitely the procedure at ski school. When I fell and had to be taken to the hospital, the ski school contacted the ski patrol before they would let my husband pick up my children!
I've never heard of it happening. As the others have said, they have a good system. You can pretty much bet that if it did happen, it would be all over the news. But, I know that doesn't mean you won't worry that it would happen to you. I am the same way. I've never left my daughter in one of those places, but looking back, I wish I had just so hubby and I could have had SOME time alone.
