It depends on the acitvity.

Most of the time, if it is with an external operator, then yes, you will be on the hook for the cost. Be aware, many excursions on Castaway Cay are also external operators.

For example, my oldest wanted to back out of parasailing last cruise. We were told that perhaps we could get a credit for a different activity from that operator, but that would need to be coordinated through the operator, not through the excursion desk. She ended up parasailing and loving it anyway, but if she decided not to go, I would have eaten the cost.

Most Disney stuff - float rentals, snorkeling gear, bikes, etc you can back out of. I think the Castaway Cay sting ray adventrure works the same way (but check on it please, I am not 100%)

Actually, you can not cancel excursions less than 3 days out. Disney will let you change excursions once onboard, but the official rule is - you can't cancel.

If you have a really good reason - like someone's sick - they will usually let you cancel.
