Not a good idea at all. Busses leaving the parks are few and far between at that time of day. You're not going to have time to do much of anything before you have to turn around and leave.
An hour should do it, as most of the people will be coming to the parks and you will be leaving. Another option is the boat to Epcot, the monorail to T&T and then monorail to the Poly. I think the bus would be quicker. If you want a sure thing, just call an Uber.

Nope. The buses don't run their normal mid-afternoon and evening routes at that time of day. OP might have to wait a long time to find a bus that's leaving the park.
That's an hour long trip at an absolute minimum, not counting transfer time and time waiting in line.

That IMO is a really bad idea. The boat ride alone will be about 30 minutes (including wait time) Unless you happen to catch the boat just as it is leaving, it's actually faster to walk. Then the entrance through the IG then the walk through all of Epcot almost. If you are lucky you might get there in an hour. You'd have a better chance waiting on the bus.
Personally I'd call Uber or a Minnie Van and call it a day. Otherwise I'd ditch the FP idea all together.
What? Wait on a boat, then walk literally 1.35 miles from IG to the front of Epcot, then wait on a monorail, then walk to the Polynesian. That's 90 minutes at least.
I would agree, I was calling the bus about an hour, but I still think the best choice in an Uber.

IMHO, if I were you, I would switch up the reservation and FastPasses time so that you go to O'hana first, and then go to Hollywood Studios. If you try to get in Fastpasses before that reservation the way you have it set now, there is a good chance you are going to run very late and possibly miss the reservation.
However, if you still want to keep said reservation, take the bus from HS to Epcot. And then hop on the monorail to the Poly. Epcot is nearby HS so it might help with the timing, but that still will be cutting it very close.
