One thing Disney could do that would help a lot is to have speed governors on the ECVs. It's easier for you to get out of their way, and easier for them to steer clear of you, when they're not going so fast.
One reason I think this argument always pertains to EVCs mostly and not wheelchairs is that wheelchairs are pushed at walking speed (usually) and are easier for the driver to steer at that speed. It's much easier to get run over by an ECV that a wheelchair, not to mention more painful.
One problem with requiring medical documentation is that you can rent these from other places and bring them into the parks. The people who don't have medical documentation will just do that. This is also an issue with speed governors since Disney can only control the Disney-rented ones.
