Disability Pass Questions

I have Lyme disease and will be traveling this trip with an electric wheelchair. I've heard from a CM that I can get a disability pass at the Guest Relations in the parks.
What is the disability pass? I have a note from my Doctor and was wondering if I would need it to show them.

A disability pass (I think they call it a convenience card or something like that) is distributed by guest relations when a guest is unable to perform normal park-required tasks (waiting in line, etc). Generally speaking, they are more for people who have an "invisible" condition... for instance, you cannot neccesarily look at a child and determine that he is autistic and therefore would be overstimulated by standing in a long-loud queue line. If it is merely a fact of being in a wheel chair, then you should be able to use the standard wheelchair entrances to different attractions. You can get a guidemap, I believe, at guest relations at each park. You might also want to look at the Passporter Guide to WDW for Special Needs (here). It is supposed to have a lot of related info.

If you do decide to try for one of the passes, you would need that doctor's note to specifically state the condition and what it prevents you from doing (standing in line, climbing in stairs, remaining in the hot sun, etc).
