Indeed, they do. However, when I did it last April, it wasn't a traditional race. It was basically "do it yourself". Start whenever you want, and whenever you finish, tell a Cast Member to get your "medal".

People are excluded because the scheduled events “sell out” and exclude kids under age 10. If it sells out, no medal. Not sure how the 2pm runner matters. They weren’t doing the 5k before Covid, either.I don't see how registering excludes anyone? The current free-for-all is a bit inconvenient for runners because the course is not closed for any length of time. Minor quibble, I know. But register and allow those who want to run early to do that, and let others do it at their leisure.
Under the current free-for-all, if you started the 5k at, say, 2pm and finished at 3:30pm and they were somehow out of medals (which I have heard does happen), wouldn't you be a bit upset? It shouldn't happen, because the medals are pretty damn cheap, all things considered, so there's no reason Disney shouldn't have boxes of them available at any given time. If you want the medal, registration is the way to go.

It just means some people can't run it at 8:30am and will run it at some point later in the day. Which is the same situation that exists now.
Everyone will be able to run it. Every single passenger on the ship will be able to run the course, even if they add a registration for people who want to run it early against other runners.
