We celebrated a birthday for my son at Disneyland last year, so it may be different. At the table service meals at Ariel's Grotto and Blue Bayou, we told the servers it was his birthday, so they brought out little treats for him.
We're going to WDW for his birthday in November, I guess I'll find out if he gets anything special then.

Not really. If the child is wearing his birthday pin, he'll likely be wished a happy birthday by many of the CMs. That's as much as you should expect. Anything more is just pixie dust and is at the discretion of the CM. At a TS meal he may or may not get a card signed by some of the characters, a free cupcake for dessert, or a balloon. My DGS was given a free Mickey bar when we stopped at a kiosk to get a soft drink. But my point is, if you go in expecting all kinds of freebies, you'll be disappointed. Go in expecting nothing but well wishes and you may be pleasantly surprised.
We've visited Disney 16 times so I know not to expect anything but we haven't gone on anyone's birthday before so I just wanted to know if there were any special events/services or anything that my 3 year old would like and that is offered on ones' birthday. Not necessarily freebies.

It really depends on where you go. Some attractions either are to busy to do anything more then wish someone a happy birthday others have time to do extra. Back a few years ago (2012) to be exact, I was there with my grown daughter and her husband and son. It was on her birthday. I wandered away from them for a few minutes and went to guest services and told them it was her birthday. They gave me the "sticker" telling everyone it was her birthday. I knew we were going to have lunch in Mexico so I asked if I could get a cake sent to the table and that I was happy to pay for it. They said, no problem, that they would set that up. When we got there the cake was brought out and was not added to the bill. We ended up over in American Adventure and a CM noticed her birthday sticker and when we got inside he came in a got her and said that there was a phone call for her and led her to the phone. It was Goofy wishing her a happy birthday. Now bear in mind this was for an adult. Something that none of us were expecting. Perhaps if we had been expecting something, it wouldn't have been enough, but, as it was it was great for her, and I, of course, got hero status for setting this up (they thought) All I did was tell them it was her birthday. And every CM that we ran into wished her a Happy Birthday. Small things sometimes mean more then the big ones.

Make sure his birthday is noted on any ADRs you may have. And make sure he's wearing his button.
Don't know if GS in Town Square still,have the telephones, it might be worth asking discreetly if he can have a birthday message. I think you can still book a wake up call from a character if you're staying onsite, but hopefully someone can confirm.

There are so many guests going to WDW with someone in their family celebrating a birthday that Disney cant do special things for everyone that has a birthday pin on. You could plan on bringing some gifts of your own and have a special celebration for your son at your resort. Buy things connected to his favorite character or Disney movie. You dont have to spend a lot since the trip is a huge birthday gift in itself. Disney does have a cake service you could order from to be delivered at your resort. Or if you did want to order something more, Disney Floral & gifts has some great birthday packages that can be ordered on their site. Weve had great service from them.

If you're eating at table service places and you tell them about it they will bring a cupcake with a candle in it. If it's a character meal they will also bring you a birthday card signed by all the characters there. That's what they did for my nephew at Garden Grill a few years ago.

They might do. My son got one at CP for his 21st. Equally they might not. It all comes down to a little pixie dust, or the luck of the draw. When people start expecting this and it doesn't happen, they get upset. So best not expect anything and be happy if anything more than lots of CMs wishing you a "happy birthday".

If you are staying onsite, and the birthday is noted on the reservation, you may get a voicemail from Mickey and the gang wishing him happy birthday.. That happened to my wife a few Years ago.. There may have been a card slid under the door overnight as well, but I don't remember..
Celebrated my birthday at Disney in 2014. Got the birthday button, a lot of well wishes, and the bus driver had all of the people on the bus sing Happy Birthday to me. I was embarrassed but thankful for the nice touch by the driver.

Just to back up what's been said, don't expect anything beyond CMs (and guests!) wishing a happy birthday.
Also in my experience the CMs in Tomorrowland are the best at saying "happy birthday." I heard it there more than in the rest of the park. Also had a CM make everyone (human) sing happy birthday to me on Carousel of Progress before it started up. Super embarrassing but also fun.
He's likely to get some stickers from CMs too, but no guarantee.
