my friend told me that you can get a fast pass if you have a chronic injury or health problem. but i can't seem to find anything online about it.

Does anyone know if this is true or not? and if so, where is the information regarding this located?

Some attractions will allow guests with wheelchairs or disabilities (and a carer) to go to the front of the line or to use another entrance to avoid waiting

If you go to guest services at each park they can provide you and your party with a pass to either allow you to go to the front of the line or wait in a seated area until it is your turn to ride the ride. We were also told to bring a letter from our physician to guest services if the illness or injury is not visible to help along the process. For example, my sister could not stand for any period of time and was pretty much wheelchair bound. We were always brought to the front of the line unless she could stay seated in her wheelchair while waiting like on It's A Small World or Snow White. Disney is very accommodating but unfortunately people have taken advantage of their hospitality therefore proof may be required.

Also note that the card you may receive from guest relations is not a fastpass, and should not be treated like one. (although some rides will send you through the fastpass line, depending on your needs)

What you're asking about is called a "Guest Assistance Card" and is by no means a "FastPass".

In some instances your wait may be longer than the standby line, in others shorter.

If you have a medical/physical condition that requires an accommodation to allow you to enjoy rides you might otherwise have to forego, they will provide you with a specific card to address your specific needs.

This is the thing I think people don't realize. There have been plenty of times that we used the wheelchair entrance on the old Star Tours where they made us sit there for 20 minutes before someone even came over to assist us.

Also, there are some injuries that don't get GAP passes. If your friend has a broken leg or is wearing the boot and is on crutches, most likely they won't get the pass.

There are also different pass, some that let you use special entrances, some that say that you are in a wheelchair or can't use stairs, others just say you can't sit in the sun.....it is totally up to the Guest Relations CM that you talk with to make that decision.

Also, with no offense to the OP as I am sure they did not know any better, but I find it rather annoying when people refer to them as fast passes or treat them as such. I cannot tell you how many times that I have been in GR and heard the person in front of me say they wanted the "handicap fast pass" so to the OP, if your friend truly needs one, make sure you do not refer to it as a fast pass or the GR CM will mostly likely tell you they have no idea what you are talking about and tell you to have a Magical Day.
