Help re changing name on passport!

Hi
going to florida in Feb and need to change my passport over to my married name.
Has anyone had to do this?
I know it is £42 but as I read the form I understood it to mean you still need two photos but they dont need to be countersigned for a name change.
Is that correct?
I presume you also have to send your marriage certificate in and your old passport?
