I guess there is one somewhere in Japan at Epcot to access the send floor restaurants.

It doesn’t quite meet the criteria but when i did a keys to the kingdom tour in the late 90s we used an elevator in fantasyland to access the utilitors

Also, doesn’t Columbia Harbor House have a small elevator in the kitchen?

Right, it was mentioned in the beginning of the thread " nothing backstage " . Because in that case there would be a ton more that guest never see like the "utilidors " which have a few. I still think we're missing some.
