That was one of the "Disney would never do this!" things I encountered at Europa Park, which I loved.
It’s very common - Dollywood, Cedar Point and Knotts are large parks that come to mind.

It just doesn’t happen at Disney.
Definitely possible. Go back and look at the Ratatouille permits. Originally it was just the ride and restrooms where the crêperie is now located. They decided to add the crêperie and changed the water management plan for the area.
Ok! I guess I thought “why do a permit just to change it” but if that’s something that happens then all bets are off.

Is it possible that they just didn’t move the railroad in those drawings cause they don’t know yet? Or would that make 0 sense?

It has always felt like Cars development was much further along then Villians, so it's possible they are holding off a decision on the railroad until they have a better idea of how Villians we be laid out.

As an analogy, I think of a restaurant being T&M, where a family can linger after the meal, and order coffee and dessert...so indeed there's incremental scope and money to be made. But it's actually better to get them out and get a new family seated (turn the table) as the yield is higher and more money will be made by the end of the night. This assumes there's a lobby of waiting families (i.e., a healthy construction market, other projects to move on to) and that's certainly not always true.
In this analogy Disney would be regular family that always orders a lot, can be demanding but also attractions other people to the restaurant. People put up with more from Disney than they would others.

Maybe they can have a walkway with a slope down, and slope up, like at Toontown at Disneyland?
They absolutely could but the permits are showing them creating obstacles to such a plan. First, the railroad isn’t level. It leaves Frontierland Station at +105’, crossed the bridge over the service canal and then descends about 10’ as it curves towards the straightaway north of Fantasyland where it is at about +95’. They’re also adding the ditch along the north side of the railroad track that drops down to +93.25’. For reference, the general level of fill called out for the river area is +98.75’. So that means you’d really want your underpass closer to Big Thunder Mountain Railroad where the railroad is highest.

The stormwater pipes being added also present a potential obstacle to future work. The last manhole before going under the r railroad has an invert at +92’ so everything in the river area needs to be above that elevation. That said, the manhole outside the tracks near the bridge location, near where the railroad is highest, has an invert at +87.25’ and is a high point, so a future underpass would be easier to drain to this area.

New fantasyland should be the expectation for this - pretty rocks, some waterfalls and streams of water, and a highly themed family “thrill” ride that should be more than it is but is still fun.
Should be the expectations for this, yes, but, THIS IS NOT THE SAME DISNEY that built New Fantasyland - See the EPCOT re do.

Villains Land looks quite small. I’m not sure how some here have hinted at 3 or 4 attractions plus retail and dining.
Disney has only indicated two attractions at this point, but it wouldn't be difficult at all to include a third if it were a smaller-footprint C. Also consider that most have been theoretically puzzling things in by utilizing the space behind Haunted Mansion and/or supposing that the coaster has a significant outdoor portion that overlaps with other elements of the land. Some have even thrown a gravity building across the tracks in their mockups. Either way, the advertised acreage is similar to TSL, which also has three attractions, a table service restaurant, and a couple snack stands despite really inefficient use of space.
